1) Gas Filter is clogged (this would generally happen more going uphill or when you accelerate)
2) Carb. is going south on you (or jets are clogged on fuel injection systems
3) Fuel Pump is dying or the lines are constricted
4) You have some debris in your gas tank which sort floats arond and cloggs the outlet from time to time
5) Your automatic choke is not functioning properly (the jerking would usually happen when the car is cold or just started running)
6) Timing slips.. or the advance mechanism on the distributor is sticky
7) Spark plug wire(s) are bad and at higher speed arcs out to the engine or ground instead of to the spark plug.
8) mechanical problem (accelerator linkage?)
9) Vacume leak (unlikely but possible)
10) Check your transmission fluid level
